Disease Therapy Management for Patients
Elwyn Specialty Care provides individualized care. Our goal is to improve our patients’ quality of life with appropriate drug therapies, and personalized support, care and compassion.
Our pharmacists and specially trained nursing staff personally review each patient's medication regimen, assist with side-effect management, and instruct patients, with printed educational material on medication and disease management. Injection training and support is provided at each of our locations or can be arranged in-home by appointment.
Medications may be picked up at Elwyn Pharmacy or delivered free to the patient’s home, workplace, or doctor's office. We will help patients with co-pay and out of pocket cost management including patient assist programs. We will encourage and enroll patients in
manufacturer support programs that will give patients access to 24/7 support. We accept all major insurance plans, including Pennsylvania, New Jersey, New York, and Delaware Medicaid.
